The Metabolic Treatment of Fibromyalgia

Order forms are provided for you below. Please see one of the order forms for the price of the book and the cost for shipping. After completing one of the order forms, transmit it to us by one of the four easy methods we describe below. All international orders (ordered from outside the USA, or to be shipped outside the USA) must be pre-paid by either draft on a U.S. bank or by VISA or MasterCard. Please send your telephone number and e-mail address with your order.

Your payment method will determine how you transmit your order to us. There are three easy ways for you to order:

A Few Final Notes For You:

Shipping & Delivery: We try to ship each customer's book within  a day or two after we receive the order. If the customer orders the book on the weekend, however, we must wait until Monday when shippers are open to take shipments. Occasionally, we must wait for a new shipment of books from the printing and binding facility before we can ship a customer's book. We try to avoid this delay, but on occasion, it occurs. We assure you, however, we'll ship your book to you as quickly as we can. If you live in the USA, FedEx delivery usually takes from 3-to-5 working days. (If we are able to ship 24-hours after receiving the order, this one day must be added to the 3-to-5 working days FedEx takes to deliver the book.) If you live in Canada or Mexico, air mail usually takes 7 days, and surface mail usually takes 10-to-14 days. Keep in mind, however, that customs sometimes holds packages for varying lengths of time before releasing them for final delivery to citizens of Canada or Mexico. If you live in Europe or the Australia/New Zealand region, air mail usually takes 10-to-14 days, and surface mail usually takes 6-to-8 weeks. Important: One of the main reasons delivery is delayed is that the customer makes an error in the information typed onto the order form. To avoid errors, please carefully review what you have typed onto the order form before you send it to us. If we must ship a book twice to a customer because the customer failed to type in the correct shipping address, we must charge the full shipping and handling rate each time. Security: For security, when we receive orders through our online credit card system, we immediately take all customer information off all our computers that interface with the Internet. The information is stored in a secured computer accessible only to authorized McDowell Publishing Company personnel. All customer information on order forms that we receive by fax or snail mail is also stored in a secured form that is accessible only to authorized personnel.
